Nearly 170m under-10s unvaccinated against measles worldwide

Figure includes half a million in UK, as Unicef warns the disease could ‘spread like wildfire’

Nearly 170 million children in the world under the age of 10, including half a million in the UK and 2.5 million in the US, are unprotected from measles in the face of growing outbreaks of the disease, Unicef is warning.

More than 21 million children a year are not vaccinated against one of the most infectious organisms in existence, says the UN body. Between 2010 and 2017, an estimated 169 million children missed the first of the recommended two-dose regime.

“The ground for the global measles outbreaks we are witnessing today was laid years ago,” said Henrietta Fore, Unicef executive director. “The measles virus will always find unvaccinated children. If we are serious about averting the spread of this dangerous but preventable disease, we need to vaccinate every child, in rich and poor countries alike.”

Simon Stevens, the chief executive of NHS England, warned the situation was serious. “Getting yourself and your children vaccinated against killer diseases is essential to staying healthy, and vaccine rejection is a serious and growing public health timebomb,” he said.

He called for Facebook and Twitter to take action against the posting of anti-vaccine propaganda and conspiracy theories. “With measles cases almost quadrupling in England in just one year, it is grossly irresponsible for anybody to spread scare stories about vaccines, and social media firms should have a zero-tolerance approach towards this dangerous content,” he said.

Measles cases are up 300% in the first three months of this year compared with the same period last year, says Unicef, with 110,000 confirmed reports. The same number died of the disease in 2017. Measles cases have reached their highest level in Europe for 20 years and caused an emergency to be declared in New York City.

Between one and three people in every 1,000 who catch measles will die, it is estimated, and there are serious complications in some who survive such as blindness, encephalitis (an infection that causes brain swelling) and pneumonia.

The US tops the list of high-income countries with the most children not receiving the first dose of the vaccine between 2010 and 2017, at more than 2.5 million. It is followed by France and the UK, with more than 600,000 and 500,000 unvaccinated infants, respectively, during the same period.

In low- and middle-income countries, the situation is critical, says Unicef. In 2017, for example, Nigeria had the highest number of children under the age of one who missed out on the first dose, at nearly 4 million. It was followed by India (2.9 million), Pakistan and Indonesia (1.2 million each), and Ethiopia (1.1 million).

Many countries have not introduced the second dose, which is given after the age of four. Twenty countries in sub-Saharan Africa do not have it in their immunisation schedule, which means that 17 million infants are at higher risk.

Dr Robin Nandy, Unicef’s chief of immunisation, said: “I’m extremely worried and everybody should be worried. I’d be very disappointed if we were not worried about it. We have had a vaccine for a number of decades now. It is inexpensive, efficacious, safe and widely available. Despite this we are seeing outbreaks all across the world.

“We know in many conflict-affected areas and remote rural areas and some urban slums there are kids being left out for whatever reason, but we are also seeing measles outbreaks in Europe, Japan and the United States and it is extremely disappointing. Measles vaccine is a silver bullet as far as public health is concerned.”

Country-wide vaccination rates can be deceptive, said Nandy. There needs to be 95% coverage to prevent outbreaks and some countries may manage 92%. But that means there will be pockets where vaccination rates are low. “There is a pool of susceptible kids concentrated in a particular location. As soon as the virus is introduced in that population, it spreads like wildfire and 90% of kids are going to get sick.”

Adam Finn, professor of paediatrics at the University of Bristol, said: “Having your child fully immunised against measles and other childhood infections should be as automatic and straightforward as teaching them how to feed themselves and sending them to school. It should be a no-brainer.

“Unicef is delivering a clear message to all of us in public service to get our act together. If we insist on playing the blame game, then we should be blaming ourselves, not parents and anti-vaxxers when things are not done right.

“It’s what we are paid to do and we have a joint responsibility to deliver. If we fail we are letting down the next generation just as negligently as by filling the seas with plastic and the atmosphere with greenhouse gases.”

Dr Mary Ramsay, head of immunisations at Public Health England, said: “These numbers highlight the importance of not only routine vaccination but also making sure anyone who missed a dose is caught up to minimise the risk of outbreaks. When you consider absolute numbers it highlights that even a tiny slip – one or two per cent different in vaccination uptake – could make a big difference.”

WHO/EU/joku voisi ostaa Plague inc.-pelin oikeudet ja sisällyttää sen osaksi opetusohjelmaa vähän kaikille koulutasoille.

Survey shows crisis of confidence in vaccines in parts of Europe

Just half of people in eastern Europe think vaccines are safe, compared with 79% worldwide

A global survey of attitudes towards science has revealed the scale of the crisis of confidence in vaccines in Europe, showing that only 59% of people in western Europe and 50% in the east think vaccines are safe, compared with 79% worldwide.

Around the globe, 84% of people acknowledge that vaccines are effective and 92% say their child has received a vaccine. But in spite of good healthcare and education systems, in parts of Europe there is low trust in vaccines. France has the highest levels of distrust, at 33%.

There have been major measles outbreaks in a number of countries, which have spread across the continent, linked to vaccine hesitancy.

Dr Heidi Larson, the director of the Vaccine Confidence Project at the London School of Hygiene and Tropical Medicine, said social media was amplifying anxieties. While confidence in vaccines in the UK was higher than in some other parts of Europe, “where the UK is now I would say is vulnerable,” she said.

The first Wellcome Global Monitor survey, which canvassed attitudes among 140,000 people worldwide, shows clear links between people’s trust in doctors, nurses and scientists and their confidence in vaccines. It also shows that mistrust in government institutions goes hand in hand with doubts about vaccines’ safety. A recent Guardian investigation looked into links between anti-establishment populist politics in Europe and anti-vax sentiment.

“This first-of-a-kind global survey clearly shows that people’s beliefs about science are deeply influenced by their culture, context and background,” said Imran Khan, the head of public engagement at Wellcome, which commissioned the survey from Gallup. “We need to care more about these connections if we want everyone to benefit from science.”

The link was clearest in the field of vaccines. “There are increasingly populations and entire countries around the world where confidence in vaccines is dropping and uptake is dropping. That does pose a huge public health risk,” Khan said.

Larson said social media had become an amplifier of doubt. “Social media is highly volatile. It has totally changed the landscape.” She said it was hard for scientists to combat the spread of misinformation because often it was not public. Much of it had shifted into private Facebook groups and other similarly inaccessible forums.

Larson said it could take years for the full effects of an incident undermining confidence in vaccines to be felt. Andrew Wakefield, the discredited gastroenterologist, published his paper claiming a link between the MMR (measles, mumps and rubella) vaccine and autism in 1998. “It was five years before his report had its full impact on vaccine uptake,” Larson said.

Bangladesh and Rwanda have the highest confidence in vaccines in the world, the Global Monitor shows. Rwanda also has the highest trust in its healthcare, at 97%, against a global average of 76%.

“In developing countries, where deadly diseases like diphtheria, measles or whooping cough are more common, I’ve seen mothers queue for hours to make sure their child is vaccinated,” said Seth Berkley, the chief executive of Gavi, the Vaccine Alliance. “It is in wealthier countries, where we no longer see the terrible impact these preventable diseases can have, that people are more reticent. This reticence is a luxury we can ill afford.”

Northern Europe, which includes the UK, has more confidence in the safety and effectiveness of vaccines than the western or eastern regions. In northern Europe, 73% of people think they are safe, the same figure as in northern America, and 84% think they are effective (83% in northern America). Only 65% of eastern Europeans think they are effective. In Ukraine, which had more than 53,000 measles cases last year, the figure is 50%.

Larson said the UK was generally doing better than other parts of Europe, but she was concerned. “I don’t think we have an emergency today, but I want to anticipate where we are going,” she said. “Trends are important. We need confidence building and information targeted at the younger generation.”

She said young parents needed to have their questions answered promptly and completely or they would look to social media instead. “There is a gap between the pace and availability of information from medical sources. People need to feel listened to. If they are not listened to, they lose trust.”

Unicef said in March that measles cases around the world were surging to “alarmingly high levels”. The disease is highly infectious and in the developing world it kills one in 100 children who contract it.

The Global Monitor found mixed feelings about science around the world. Half the world’s population said they knew little about science and 20% said they felt excluded from its benefits. Fifty-five per cent of people in France said they believed science and technology would lead to job losses in their area, as did 37% of people in the UK.

Meet the New York couple donating millions to the anti-vax movement

Image
A wealthy Manhattan couple has emerged as significant financiers of the anti-vaccine movement, contributing more than $3 million in recent years to groups that stoke fears about immunizations online and at live events — including two forums this year at the epicenter of measles outbreaks in New York’s ultra-Orthodox Jewish community.

Hedge fund manager and philanthropist Bernard Selz and his wife, Lisa, have long donated to organizations focused on the arts, culture, education and the environment. But seven years ago, their private foundation embraced a very different cause: groups that question the safety and effectiveness of vaccines.

How the Selzes came to support anti-vaccine ideas is unknown, but their financial impact has been enormous. Their money has gone to a handful of determined individuals who have played an outsize role in spreading doubt and misinformation about vaccines and the diseases they prevent. The groups’ false claims linking vaccines to autism and other ailments, while downplaying the risks of measles, have led growing numbers of parents to shun the shots. As a result, health officials have said, the potentially deadly disease has surged to at least 1,044 cases this year, the highest number in nearly three decades.

The Selz Foundation provides roughly three-fourths of the funding for the Informed Consent Action Network, a three-year-old charity that describes its mission as promoting drug and vaccine safety and parental choice in vaccine decisions.

Lisa Selz serves as the group’s president, but its public face and chief executive is Del Bigtree, a former daytime television show producer who draws big crowds to public events. Bigtree has no medical credentials but holds himself out as an expert on vaccine safety and promotes the idea that government officials have colluded with the pharmaceutical industry to cover up grievous harms from the drugs. In recent weeks, Bigtree has headlined forums in ultra-Orthodox Jewish communities in Brooklyn and Rockland County, N.Y., both areas confronting large measles outbreaks.

“They should be allowed to have the measles if they want the measles,” Bigtree told reporters outside the Brooklyn meeting on June 4. “It’s crazy that there’s this level of intensity around a trivial childhood illness.”
